Request to File A Complaint


Please use this complaint entry process when requesting to file a complaint. If the contractor is licensed, it is important that you know the correct name and/or license number. If the contractor is unlicensed, the project cost must be $40,000.00 or more to be within the Board’s statutory authority.

DO NOT FILE AN ELECTRONIC COMPLAINT REQUEST IF YOU HAVE ALREADY SUBMITTED A PAPER COMPLAINT TO THE BOARD’S OFFICE BY MAIL OR E-MAIL.

DO NOT USE THIS FORM FOR SUBMITTING ADDITIONAL INFORMATION FOR A COMPLAINT THAT HAS ALREADY BEEN FILED.

Please briefly provide the requested information and attach any other documents you feel may be relevant in determining whether or not your complaint may be within the Board’s jurisdiction.  The Board has limited authority over its licensees.    Submitting this complaint request form does not mean you have filed a formal complaint in accordance with the Board’s rules and regulations.

Once your complaint request summary has been reviewed, you will be contacted by a staff member and may be provided additional forms to complete the complaint process. NOTE: Time to complete this form will expire after 15 minutes.

The Complaint Request Process will lead you through the following steps:

  1. Identify yourself (the Complainant )
  2. Identify whom the Complaint is against (the Respondent)
  3. Provide details about the problem
  4. Provide supporting documents (optional)
  5. Identify any supporting witnesses (optional)
  6. Finalize the Complaint Request

NOTICE: The Licensing Board has the authority to discipline a contractor’s license, the Licensing Board does not have the authority to require a contractor to pay money, make restitution, compel the contractor to complete unfinished projects or to undertake remedial work.
